Bumrungrad International Hospital Services and Benefits 
• One way airport pick up to hotel (BKK/DMK)
• Guest will be tested COVID-19 at hotel on day 5 and day 12
• 24-hour video consult service for COVID-19 concerns
• Bumrungrad telehealth service
• Surgical face mask, hand gel, thermometer for 16 days 

Package Rate (net) Single Stay for 15 nights starting THB 53,000 net/person/room
• Rates are net & inclusive of 10% service charge and 7% VAT
• Room rates valid for all arrivals before 30 June 2021 

Other Services (optional with extra charge) 
• Extra child under 12 years old and spouse with marriage certificate charged at THB 40,000
• Laundry service package - 45 pieces for 15 days at THB 2,250 net (excludes dry cleaning)

Terms of Payment 
•    Full payment in advance before arrival

Cancellation and No-Show Terms & Conditions 
• Cancellation is permitted up to 14 days prior to arrival
• Cancellation charge 50% from total amount will be applied for late cancellation less than 14 days prior to arrival
• A cancellation charge of 100% of the total amount will be applied for late cancellations less than 7 days prior to arrival
• If a guest tests positive for COVID-19 during their stay at the hotel, the full amount charged will not be refunded.

Important notice:    
Upon the receipt of the booking notification, we would like you to send back all the below required documents to ensure your 
room reservation is reserved. Without all the accurate required documentation, we may not be able to accept your room 
reservation. The final confirmation of the booking will be immediately submitted once all the documents are completely checked and 
validated.
Kindly provide us with the below documents: 
1.    Flight itinerary (Original departure flight number) 
2.    COE document from Embassy
3.    1 inch photo
4.    Travel insurance
5.    Photocopy/scan of passport for all nationalities (According to government regulations, please do not sign or mark on the copy)
6.    Medical certificate of COVID-19 examination results 
Kindly provide us with the following information:
1.What is the city of the embassy you are registered with? Example: Royal Thai Embassy in Washington D.C.
2.Dietary restrictions 
3.Address in Thailand for long term stay after quarantine 
4.Emergency contact (Name, relationship and phone no.)
5.Name and address (TAX ID) to issue invoice/receipt
Once you receive your booking notification email, you will also receive a separate email from our reservations team who
will send you several forms for you to fill in. Kindly return the completed filled forms as soon as possible.

4.Informed consent for risk of sharing the same room (For families)
5.Informed consent for risk of guests without health / life insurance coverage for COVID-19 and guests without pre-boarding RT-PCR COVID-19 swab results
